Through this form, we collect your name, email address, phone number (if provided), country and region of residence, your relationship with Monolithic Power Systems Inc. ("MPS"), the nature of your request, any supporting details you provide, and any identity verification documents you upload. This data is processed solely to verify your identity and fulfill your privacy request. To protect your privacy, we are required to verify your identity before processing your request. The level of verification depends on the type of request and applicable law. Depending on your request, we may ask you to confirm information we already hold about you (such as recent orders, account details, or prior interactions, or provide government-issued identification (which will be used solely for verification, processed in accordance with our Privacy Policy, and securely deleted within 30 days of verification.

If you have enabled a universal opt-out preference signal such as Global Privacy Control (GPC)(https://globalprivacycontrol.org) in your browser or device, we honor that signal as a valid opt-out of the sale or sharing of your personal information and/or targeted advertising, as required by applicable law. You do not need to submit this form separately to exercise that right if you have already enabled such a signal.
